Medicare
If you are on Medicare you will need to sign a contract that Medicare requires. We will provide you with that contract on your first visit.
Scheduling
If a patient needs to change or cancel their appointment, a 2 business day notice is required. This means that if the appointment is on Friday, the patient would need to call by Tuesday night. This would give us two full days to fill that appointment.

The Next Step
In the interest of providing the most accurate evaluation and diagnosis of your condition, we must see you in person. Some follow-up consultations may be by phone, however personal visits are recommended.
